  • Prescription Renewals

    Prescriptions are renewed during regular office hours. In some instances, we may require that the patient be seen in our office prior to medication renewal.

Via Sunrise Highway:

Take Sunrise Highway to Grand Avenue in Baldwin. Coming from the east, make a left onto Grand Avenue (Baldwin train station will be on your right). Coming from the west, make a right onto Grand Avenue (immediately past Best Buy Shopping Center which will be on your right). Proceed south on Grand Avenue and follow directions via Grand Avenue, below.

 

Via Southern State Parkway:

Take Exit 20 South-Grand Avenue, Baldwin. Continue south on Grand Avenue. After approximately 2 miles, you will pass under a rail trestle. Cross over the intersection at Sunrise Highway and follow directions via Grand Avenue, below.

Via Grand Avenue:

From Sunrise Highway, proceed south on Grand Avenue. Cross the intersection at Merrick Road and bear left at the fork to continue on Grand Avenue. At the second traffic light turn left onto Atlantic Avenue. Dr. Jonathan Altus’ office is immediately after the second traffic light, located on the right side, opposite Waldbaums Shopping Center. Please park in the adjacent parking lot.
